You've Got a Clear Mind and Solid Goals, but There's Just One Problem... Time.
While you have set these new goals and priorities, odds are you still have a full schedule to attend to such as a job, or kids, and setting new goals didn't exactly subtract from that... Now you have to find the extra time to spend on achieving these goals, while still managing your responsibilities.
This is the other big reason why goals remain un-achieved - either they were built on shallow intentions, or you couldn't focus your efforts and manage your time effectively enough to accomplish what you needed to, when you needed to.
In the end, it honestly all comes down to the fact that we all make time for what we want to do and what we find important. This is why it is imperative to work on developing powerful "why's" - you have to have deep, intrinsic motivators so that you naturally make working towards your goals a priority.
If time is a major concern for your ability to maintain a fitness regiment, one of two things need to happen - you either need to reevaluate your motivators for wanting to improve your fitness, or you need to reconcile your time better.

You need to reconcile your time as you would your checkbook;
You have to know where you are spending your most valuable resource. Seriously... reconcile your time. For 3-7 days keep track of how you spend every minute; 30 minutes for breakfast, 1 hr for dinner (cooking and for the meal) 2 hours total on Facebook, 3 hours reading email, etc.
You will probably be amazed at what this reveals - the times I listed are not too far off base for the average American, and that is a very basic list of examples.
As you track these, take not of anything that you notice is a consistent time killer and look at ways of reducing it.
Using Social Media or Email as an easy example - if you note that you spend way too much time on either or both of these; schedule set times for them - 30 minutes or 1 hour Max on social media, 1 hour first thing in the morning for reading/answering email, etc.
From now on, anything that is not directly contributing towards achieving your goal is a non-factor for you - if you haven't done everything possible to get your closer to your goal, then these inappurtenant distractions don't get a second of your attention.

Once you begin to ingrain this type of thinking into your life, that is the start of developing "front-sight focus". Front-sight focus is technically a military term used to teach the proper method for lining up their weapons to fore (and hit) targets at impressive distances consistently.

You focus only on the front sight post. Everything else is slightly out of focus and you align them with the front sight.
The rear sight and target are both out of focus, you align them to your primary point of focus to get a perfect alignment to your target.
This gives you a single point of focus vs. having multiple to deal with. When you see a special forces team engaging what appears to be multiple targets at once, what they are actually doing is focusing on one at a time, but because they are so focused they are able to deal with one and move to the next quickly, making it seem as though they are engaging multiple targets at once.
